Duratiion Type	Optional field used to identify the duration of exposure type.
Hazard Type*	Users can select from Chemical/Panel or Physical. Mandatory field.
Chemical	Optional field used to select the chemicals.
Panel	Optional field used to select the panel.
Group size (N=)*	Enter the number of people in the group. Mandatory field.
Exposure Group Description	Optional text field to enter in any addtional information about the exposure group.
