The application has been enhanced to provide the ability to capture why, in specific, the applicable regulatory agency was not notified.

...

• New field of “Please Explain why applicable regulatory agency was not notified” has been introduced.
• This field will be displayed only when the response to the question “Was the applicable regulatory agency notified?”
• The subsequent field of “How was the Agency Notified”will only be displayed when a 'Yes' is responded to question "Was the applicable regulatory agency notified".
