Who:
- Establishments with 250 or more employees that are currently required to keep OSHA injury and illness records, and
- Establishments with 20-249 employees that are classified in certain industries with historically high rates of occupational injuries and illnesses.

OSHA 300A E-Submission
Ensuring the Accuracy of the OSHA 300A Summary Data
- Whether generating the CSV file for a single establishment or multiple establishments via the 300A screen, all applicable establishments must first have the 300A screen completed and saved for the applicable reporting year.
- Company Name
- The name of the company that owns the establishment
- 100 Character Limit
- Establishment Name
- Each establishment MUST have a unique name
- 100 character limit
- Employer Identification Number (EIN)
- 9 digit number
- Do not include dashes
- Not required for 300A Summary, but is required for OSHA ITA submission
- Street
- The street address of the establishment
- 100 Character Limit
- Do not enter multiple lines
- Do not list PO boxes
City
The city where the establishment is located

State
The state where the establishment is located
State selected from drop-down (Converted to 2 characters in the CSV file, per OSHA requirement)
- Zip
- The full zip code for the establishment
- Must be a number and five or nine-digit in length (No text)
- If a hyphen is included, it must be manually removed from the CSV file prior to upload in OSHA’s ITA
- If the zip code begins with a zero, it must be manually added in a CSV file prior to upload
- Industry Description
- Provide an industry description in addition to your NAICS code
- 300 character limit
- NAICS
- The North American Industry Classification System (NAICS) code which classifies an establishment’s business
- Must be a number and be 6 digits in length
- Do not list multiple NAICS codes.
- Establishment Type
- Identify if the establishment is part of a state or local government
- Default value: Not a Government Entity
- Converted to a numerical value in the CSV file, per OSHA requirement
- Establishment Size
- The size of the establishment based on the maximum number of employees which worked there at any point in the year you are submitting data for
- Select Establishment Size from the drop-down
- Converted to a numerical value in the CSV file, per OSHA requirement.
Year
The calendar year in which the injuries and illnesses being reported occurred at the establishment
Select Reporting Year from the drop-down
- Annual Average Number of Employees
- The average number of employees for the reporting year
- Converted to a numerical value without decimal in the CSV file, per OSHA requirement
- Total Hours Worked by All Employees
- Total hours worked by all employees for the reporting year
- Converted to a numerical value without decimal in the CSV file, per OSHA requirement.
- Injuries and Illnesses*
- Whether the establishment had any OSHA recordable work-related injuries or illnesses during the year
- 1 populated for yes in the CSV file
- 2 populated for no in the CSV file

Total number by case type: Cases involving Death, Days Away from Work, Job Transfer or Restriction, and Other Recordables

Numbers populated by case type in the CSV file.

Generating OSHA 300A CSV for Submission
Single Establishment via OSHA 300A Screen

- Navigate to OSHA 300A screen and navigate to the desired node from Navigator
- OSHA 300A will display list of locations where the form is saved for the reporting year
- Select locations for which CSV needs to be generated
- Click on Generate CSV File button
- Save downloaded CSV file
- Review CSV prior to submission in OSHA’s ITA site
- Submit via OSHA’s ITA site: https://www.osha.gov/injuryreporting/ita/
OSHA 300A Submission via OSHA API
Single Establishment via OSHA 300A Screen
- Review OSHA 300A in entirety then Save
- Select Submit Using OSHA API button
- Login using credentials registered/ created on OSHA ITA site
- Not your ProcessMAP credentials
- Reset OSHA ITA Credentials
- Acknowledge the disclaimer and Click on Submit button
- System will show Success message if no error in the data.
