  1. Once you are homepage, you can click on the “Incident Management” module shortcut.

  2. Type in the Incident Title/Site IMS* and Check off the desired Incident Type

  3. Once the Incident Type is selected, the Incident Details section will update with questions based on the Incident Type(s).

  4. Select the the YES or NO button for question: Were multiple people injured as part of this incident?

  5. Enter the Information in Fields and Once all required fields are filled, save it.

1. Currently the Investigation is Incomplete as its not started and Case Management and Investigate sections are Empty and We can also create Corrective Actions / action items in the same page.

2. Click on Incomplete to Start the Incident Investigation

3. Enter the Information in Fields and Once all required fields are filled, save it.

4. Click Next to procced to Investigation Step.

5. Click on New to Add Contributing factors and Root Cause Analysis.

6. Select Contributing Factors from the Pick List Once all required fields are filled, Select it.

7. Select 5 Why’s and can also add more reasons to contributing factors.

8. Select the Root Cause by doing Root Cause Analysis for the Incident to determine basic root cause Once all required fields are filled, save it..

9. Click on New to Add Action Items

10. Fill the required Action Items for for Corrective, Preventative and Containment Action. Once all required fields are filled, Save it. This is will save the Action Item and Notify the Action Item Owner.

11. Investigation is now Saved and Complete and Lessons Learned Statement can be inserted in Tab Below.

12. Next Select Case Management and fill the required fields and Incident Investigation is complete.
