Step-by-Step Instructions:

Reporting a Near Miss Incident

(info)(info) In order to create an incident record user must navigate to a site/location Image Addedto see the Report New Incident menu option.

Navigate to: Desired Location or Level >  > Modules > Incident Management > Report New Incident

6. Click the Save button.

  • After saving and completing the Incident Detail Report the Incident Summary screen will be displayed. 
  • Users can then access the other components from the incident detail summary screen.  Refer to: Near Miss Detail Summary

 Injury/Illness vs. Near Miss

  • There are two types of work related incidents which can be reported – Injury/Illness and Near Miss.  

    • Injury/Illness incident someone was injured or became ill.

    • Near Miss incident someone could have been injured or become ill.

4. Select Near Miss in Incident Type.

5. Fill all the Mandatory Fields and Click on Save button.
