User Story #1 : As an EHS Head, I would like to report all injury and illnesses for recordkeeping purposes.

User Story #2 : The Incident Management module allows managers or As a EHS specialists I would want to manage the lifecycle of incidents. Incident reporting This involves a multi-step process where incident details are captured and then an investigation is performed.  Once the reason of an occurred incident occurred is identified, corrective / preventative actions (CAPAs) are needs to be created to address problems so that similar incidents are not as likely to occur again in the future.  

👥 What am I doing now ?

I would have to create new Incident by clicking on report new Incident in Incident management drop down tabrecord incidents on documents and then conduct investigations on them on standard documents or spreadsheets.

  2. Metrics- The Metrics component gives the count of the number of incidents of all types based on different criteria.

  3. Ability to generate OSHA Reports with few clicks.

Reporting an Incident

  1. Once you are

  1. logged into the system, click on the Incident Management Icon, this will land you on to the Incident List screen.

    Click on the Report button to create a new incident, Incase you do see the button, please make sure you are at a Location in your navigator.

  3. This would open up the form, select the incident type and fill out the form.

  4. Image Added

    Make sure all the single asterisk & double asterisk fields are filled before saving the form.

  5. You will be now redirected to the Summary page, you can check if the initial form is completed by checking the status icon right next to Report label.

  1. Once you are homepage, you can click on the “Incident Management” module shortcut.

  4. Type in the Incident Title/Site IMS* and Check off the desired Incident Type

  5. Once the Incident Type is selected, the Incident Details section will update with questions based on the Incident Type(s).

  6. Select the the YES or NO button for question: Were multiple people injured as part of this incident?

  7. Enter the Information in Fields and Once all required fields are filled, save it.

1. Currently the Investigation is Incomplete as its not started and Case Management and Investigate sections are Empty and We can also create Corrective Actions / action items in the same page.

2. Click on Incomplete to Start the Incident Investigation

3. Enter the Information in Fields and Once all required fields are filled, save it.

Conducting Investigation

Investigation in system is a multistep process.

  1. Answering to Investigation questions (Optional)

  2. Identifying Contributing Factors

  3. Identifying WHYs for selected Contributing Factor

  4. Choosing Root Cause among WHYs and classifying root cause.

  5. Root Cause Analysis and Action Plan recommendation.

  6. Final Root Cause Statement

  7. Lessons Learnt

Answering to Investigation questions

This step is optional depending upon your system’s current configuration. As an investigator, you would have to answer few additional questions.

You can do that by clicking on Investigation details section.

Identifying Contributing Factors

  1. Click on the contributing factors link to open up the option to select contributing factors

2. Click on new to select the contributing factor(s) for the incident.

3. From the picklist, select one contributing factor.

Identifying WHYs for selected Contributing Factor

  1. Once the contributing factor has been selected, you can identify the WHYs for them.

2. Once the Contributing factor is selected and whys are added, click on the “Save” button.

3. Repeat the whole process, if you have more than one contributing factor.

Choosing why Root Cause among WHYs and classifying root cause

  1. Now that you have identified the Contributing factor and WHYs, you can choose a why root cause among the WHYs

  2. This can be done by clicking on checkbox right next to WHY and click on Save button.

    Now we move on to Root Cause Analysis and Action Plan recommendation.

Root Cause Analysis and Action Plan recommendation

  1. Open up the Root Cause Analysis section, this would show the selected contributing factor , WHYs and why root cause.

  2. Now select the Standard Root cause against each of the why root cause.

    Once the standard root cause is chosen, you can create Action Plans.

    This would create Action plans for Root causes.
