This document will demonstrate how to create an a Near Miss incident record.  The Near Miss incident type is an unplanned event that did not result in injury, illness, or damage – but had the potential to do so.

  2. Check off the Near Miss box for your Incident Type*

  • After saving and completing the Incident Detail Report the Incident Summary screen will be displayed.

 Injury/Illness vs. Near Miss

  • There are two types of work related incidents which can be reported – Injury/Illness and Near Miss.  

    • Injury/Illness incident someone was injured or became ill.

    • Near Miss incident someone could have been injured or become ill.

4. Select Near Miss in Incident Type.

5. Fill all the Mandatory Fields and Click on Save button.
