Step-by-step guide
- Click on the Modules Icon.
- Click on the Incident Management Menu Item.
- Click on the Manage Incident Reports Menu Item.
4. Select the Incident from the Incident List.
5. Select the required 5 Why? Methodology that need to be completed, by clicking on the Incomplete Hyper Link.
6. Click on the Add Evaluation Tab.
7. Fill in the Incident Details under 5 Why? Methodology, screen.
A. Select the Contribution Factors from the list
B. Please check the check box if it is a FINAL ROOT CAUSE.
C. Click on the Save, Button.
8. Click on the OK, Button.
