1. Click on the Modules Icon icon.
2. Click on the Incident Management Menu Item menu item.
3. Click on the Manage Incident Reports Menu Item menu item.

4. Select the Incident incident from the Incident List. 

5. Select the required Root Cause Statement section to be completed , by clicking on the Incomplete hyperlink.

6. Select the Root Causesroot causes.

7. Click on the Save button.
