- Click on the the Modules Icon icon.
- Click on the the Incident Management Menu Item menu item.
- Click on the Manage Incident Reports Menu Itemthe Manage Incident Reports menu item.

- Click on the blue hyperlink Incident ID from the Incident List screen.

5. Select the required Root Cause Statement section to be completed, by clicking on the Incomplete hyperlink.
6. Select the Root Causes.
7. Click on the Save button.

- In Investigate section, click on the "Incomplete" hyperlink near Root Cause Analysis.
- Select the Root cause statement.
- Click on the Save Button.
