This document will demonstrate how to create an Near Miss incident record.

Reporting a Near Miss Incident

(info) In order to create an incident record user must navigate to a site/location to see the Report New Incident menu option.


Navigate to: Desired Location  > Modules > Incident Management > Report New Incident

  1. Type in Incident Title / Site*

  2. Check off the Near Miss box for your Incident Type*

  • After saving and completing the Incident Detail Report the Incident Summary screen will be displayed. 
  • Users can then access the other components from the incident detail summary screen.  Refer to: Near Miss Detail Summary

Allows the user to search for an employee

        

 

 

After saving the incident report the incident detail summary screen will be displayed. Users can access the other components for the incident from the incident detail summary screen.

Related issues

 Injury/Illness vs. Near Miss

  • There are two types of work related incidents which can be reported – Injury/Illness and Near Miss.  

    • Injury/Illness incident someone was injured or became ill.

    • Near Miss incident someone could have been injured or become ill.


4. Select Near Miss in Incident Type.

5. Fill all the Mandatory Fields and Click on Save button.
