Step-by-step guide
- Click on the "Modules" Icon.
- Click on the "Incident Management" Menu Item.
- Click on the "Manage Incident Reports" Menu Item.
4. Select the Incident from the Incident List.
5. Select the required 5 Why? Methodlogy that need to be completed, by clicking on the "Incomplete" Hyper Link.
6. Click on the "Add Evaluation Tab".
7. Fill in the Incident Details under "5 Why? Methodlogy", screen.
A. Select tthe Contribution Factors from the list
B. Please check the check box if it is a FINAL ROOT CAUSE.
C. Click on the "Save", Button.
8. Click on the "OK", Button.
