Step-by-step instructions:
- Click on the Modules icon.
- Click on the Incident Management menu item.
- Click on the Manage Incident Reports menu item.
4. Select the incident from the Incident List.
5. Select the required 5 Why? Methodology that needs to be completed by clicking on the Incomplete hyperlink.
6. Click on the Add Evaluation tab.
7. Fill in the incident details under 5 Why? Methodology screen.
A. Select the contribution factors from the list
B. Please check the check box if it is a FINAL ROOT CAUSE.
C. Click on the Save Button.
8. Click on the OK Button.
