Reporting a Near Miss, Unsafe Act, Unsafe Condition

This document will demonstrate how to create a Near Miss incident record. The Near Miss incident type is an unplanned event that did not result in injury, illness, or damage – but had the potential to do so.

Reporting a Near Miss, Unsafe Act, Unsafe Condition

(info) In order to create an incident record the user must navigate to a site/locationto see the Report New Incident menu option.


Navigate to: Desired Location  > Modules > Incident Management > Report New Incident  Refer to: Reporting New Incident Overview 

  1. Type in Incident Title / Site*

  2. Check off the Near Miss box for your Incident Type*

information (i) Near Miss and General Liability incident types are stand-alone incident types and cannot be combined with other incident types.

3. Complete the following sections:

      a. General Details

      b. Employee / Individual Details

      cWitness Information (Optional)

4. Click the Save button.

 information (i)  (*) Indicates that it is required in order to Save.  (**) Indicates that it is required in order to generate notifications and complete section.

(lightbulb) If Yes is selected the following questions are added:

Personnel Type* has the following options available:

  • Employee

  • Supervised Contract Employee

  • Unsupervised Contract Employee

  • Visitor

Employee / Individual Involved* allows the user to search for any employee.

  • The search box will bring up only employees based on location selected.
  • The picklist button will search for employees at other locations.


(tick) Next Step:
  • After saving and completing the Incident Detail Report the Incident Summary screen will be displayed. 
  • Users can then access the other components from the incident detail summary screen.  Refer to: Near Miss Detail Summary

Note

Email Incident Summary report will be available Once user saves the incident.

 

18.3 Update

  • For mandatory fields in the particular detail page, Click on the  icon near Incident - Detail Report
  • All the Mandatory fields are appeared.



  • Allows to save an Incident with Minimal information
  • Incident status will remain as Draft until all mandatory fields are filled.
