The Questionnaire tab is only available for protocol driven audits. The Questionnaire tab allows users to record the answers to questions, report findings for non-preferred answers, create action items, enter comments and upload attachments. The questions are organized by section. For protocol driven audits, this is the key component for entering data.

Response Types
| Response | Definition |
|---|---|
Preferred | Indicates there is no issue found for the question and no finding will be created. |
Non-preferred | Indicates a deficiency was identified during the audit. A finding and at least one action item will need to be created. |
Ignore | Indicates the question does not apply to the location being audited.
