Questionnaire Tab

The Questionnaire tab is only available for protocol driven audits. The Questionnaire tab allows users to record the answers to questions, report findings for non-preferred answers, create action items, enter comments and upload attachments. The questions are organized by section. For protocol driven audits, this is the key component for entering data.

The Questionnaire tab can be accessed for protocol audits under the Perform Audit tab as shown below:

Step-by-step Instructions:

Accessing the Questionnaire for a Protocol Driven Audit

Click on the Modules icon.
Click on Audit Management menu item.
Click on Manage Audits menu item.
Click on the Name of the Audit Program.
Click on Perform Audit tab.
Click on the Questionnaire tab.

Response Layout

The --Select-- drop down contains the question responses: Prefered, Non-Prefered and Ignore.

Each question has a icon which allows users to enter a finding if a non-preferred response is chosen. Users can enter action items, comments and attachments for all questions.

The menu before a response is selected is shown below:

Response

Definition

Preferred

Indicates there is no issue found for the question and no finding will be created.

Non-preferred

Indicates a deficiency was identified during the audit. A finding and at least one action item will need to be created.

Ignore

Indicates the question does not apply to the location being audited.

If scoring is being used, a question answered with a response type of ignore will not be included in the calculations

Questionnaire Icon Meaning

Under Audit Setup, Program details tab, Advanced Details section, New field of Require root cause analysis has been introduced in the audit set up tab.
If the response to this field is yes then the Root cause icon will be displayed in the Questionnaire.Else the root cause icon will not be displayed in the Questionnaire screen
Irrespective of the response to this field ,the user would be able to enter root cause in the findings list (Manage Findings and Findings Summary).

Under Audit Setup, Program details tab, Advanced Details section, New field of "Create findings while responding Audit Questions" has been introduced in the program details page .
If the response to this field is yes then the finding icon will be displayed in the questionnaire.Else the finding icon will be hidden in the questionnaire.
