The Questionnaire tab is only available for protocol driven audits. The Questionnaire tab allows users to record the answers to questions, report findings for non-preferred answers, create action items, enter comments, and upload attachments. The Questions are organized by section. For Protocol driven audits this is the key component  for entering data.

  • Auditors can skip an entire section of questions by checking the box next to Not Applicable for the Section Name.

  • If any changes were made to the questions within the Protocol, users can reload the audit protocol by clicking on the Image Removed button.

The questionnaire tab can be accessed for protocol audits under the perform Audit tab as shown below:
