Adding an Action Item to Questions in the Questionnaire tab.
Navigate to: Modules>>Audit Management>> Manage Audits>>Select Audit Name (Refer to Perform an Audit)
- Navigate to the Questionnaire tab from the Perform Audit section of the selected audit.
- Click on the Action Item icon
for the selected question.

Image Added - Click on the Add button on the upper right of the Action Item List screen.
