The Questionnaire tab is only available for protocol driven audits. The Questionnaire tab allows users to record the answers to questions, report findings for non-preferred answers, create action items, enter comments, and upload attachments. The Questions are organized by section. For Protocol driven audits this is the key component for entering data.
Note:
- Auditors can skip an entire section of questions by checking the box next to Not Applicable for the Section Name.
- If any changes were made to the questions within the Protocol, users can reload the audit protocol by clicking on the button.
The questionnaire tab can be accessed for protocol audits under the perform Audit tab as shown below:
The response types include the following:
- Preferred – indicates there is no issue found for the question and no finding will be created
- Non-preferred – indicates a deficiency was identified during the audit. A finding and at least one action item will need to be created
- Ignore – indicates the question does not apply to the location being audited. If scoring is being used, a question answered with a response type of ignore will not be included in the calculations
Each question has a menu which allows users to enter a finding if a non-preferred response is chosen. Users can enter action items, comments and attachments for all questions.

Accessing the Questionnaire for a Protocol Driven Audit
- Click on the Modules icon.
- Click on Audit Management menu item.
- Click on Manage Audits menu item.
- Click on the Name of the Audit Program.
- Click on Perform Audit tab.
- Click on the Questionnaire tab.
