Questionnaire attachments are used to store any attachments to a question.

Step-by-step Instructions: 

Adding Questionnaire Attachments:                                                                       Accessing/Viewing Questionnaire Attachments

Navigate to: Modules>>Audit Management>> Manage Audits>>Select Audit Name   (Refer to Perform an Audit)  

  1. Navigate to the Questionnaire tab from the Perform Audit section of the selected audit.
  2. Click on the Attachments icon  located to the right of the selected question.


  3. Click on the Add button located at the bottom right of the Attachments screen.


  4. Click on the Browse button.
  5. Select a file to upload. 
  6. Click on the Open button. 


  7. Click on the Click to Attach File button. 


  8. Complete the Document Details screen. 
  9. Click on the Save button. 


  10. Click on OK button from the Success confirmation window. 


  11. Click on the Close button at the bottom right of the Document Details screen.


    The attachments iconwill turn greenonce the attachment(s) have been successfully saved.

  12. Click on the Close button from the Attachments screen.

Accessing/Viewing Questionnaire Attachments:  

  1. Access the Attachments screen by clicking on the Attachments icon  located to the right of the selected question.


  2. Click on the icon located under the View column for the desired attachment in the Attachments screen to open it.
