Step-by-step instruction:

  1. Click on the Modules icon.
  2. Click on the Audit Management menu Item.
  3. Click on the Manage Audits menu item.



  4. Click on the Name of the Audit Program.



  5. Click on the Perform Audit tab.
  6. Click on the Questionnaire tab.
  7. Click on the Action Item icon.



  8. Click on Add button.



  9. Complete all the fields and click on the Save button.
