PMP-56614 As a System Admin I would like to have workflow for Rejected Management Review

Configurable: No

  • Title: NA
  • Public / Private: NA
  • Default: NA

Existing System

  • When Management Review is rejected by a Role system does not trigger any notification to stake holders
  • When Management Review is rejected by a Role system does not take the flow back to previous step
  • Intimation Notification does not include Comments provided at time of Approval/Rejection

Purpose:

  • Keep all stake holders informed in case of rejection and take the process to previous step for required action

Enhanced System:

  • When a role Rejects Management Review, previous Management Review role (where status is Complete) status will be set to Re-Open
    • The Re-Open status is not available for manual selection
  • Intimation Notification will be triggered for owners of previous role
  • If Management Review is rejected by the 1st role then notification would be triggered to Investigation Team by default
  • If current Role status is "Reopen", the owner of next role should not be able to perform management review irrespective of mandatory or optional i.e. hide Save button for the next role.

PMP-56616 As a System Admin I want to lock certain fields in FROI and Manage them from Investigation Details

Configurable: Yes

  • Title: Display Froi fields in Investigation Questions Section
  • Public / Private: Private
  • Default: Product: Disabled. Enabled only for FCA

Existing System

  • Does not have information flow from FROI to Investigation Section and vise-versa 
  • Does not have ability to lock certain fields in FROI based on Investigation Details status

Purpose:

  • Provide ability to correct only few fields information without giving access to FROI

Enhanced System:

  • Displays Incident Description, Department and Location of Injury scene information in Investigation question
  • Provides ability to update the information from Investigation Details screen without the need of having Edit Incident permission
  • For FCA custom settings related to above fields are set to display these fields and made Private to avoid accidental change in configuration

PMP-56613 As a System Admin I want to select Investigation Responsibility Team as Management Review owner

Configurable: Yes

  • Title: InvestigationResponsibilityShowYN (Existing)
  • Public / Private: Private
  • Default: Disabled. Enabled for FCA

Existing System

  • As of now system does not allow to select a predefined investigation team in Management review
  • Each of the users need to be selected once again manually.
  • Any change in the investigation team needs to be updated in the Management review

Purpose:

  • Make it easy to assign and manage Management Review owners for cases where Investigation Team is the 1st level Management Review team

Enhanced System:

  • Has a checkbox available to include Investigation Responsibility Team as Owner in Module Setup as well as Incident Summary >> Role Setup
  • If the checkbox is checked for a Role, Investigation team will be included as owners in the Role when Management Review starts
  • Any change made in Incident Investigation responsibility team after Management review has started will reflect in Management review once the Role is saved again
  • For Management Review Setup screen from Incident Summary, if the check box is unchecked, respective Investigation Team user names will not be removed from Primary owner field. Un-checking the field will only prevent any changes in Investigation Team to reflect in the role

PMP-56407 As an end user I would like to generate SEWO report after detail report status changes to Incomplete

Configurable: No

  • Title: NA
  • Public / Private: NA
  • Default: NA

Existing System

  • As of now SEWO report link is enabled only when Investigation section status is changed to Complete as most of the information is coming from Investigation section

Purpose:

  • To be able to generate and print the report for manual usage

Enhanced System:

  • System allows to download the report for manual usage as soon as an Incident is created (status as Detail report Incomplete).

Impacts:No Impact

PMP-56497 As an end user I would like an option to upload Image for SEWO report sketch box

Configurable: Yes

  • Title: SEWO Report (existing)
  • Public / Private: Private
  • Default: Disabled in Product

Existing System

  • Does not have an interface to upload SEWO sketch image in system

Purpose:

  • Provide ability to upload SEWO sketch image to be displayed in SEWO report

Enhanced System:

  • Provides an interface to upload image specific to SEWO report
  • Allows to upload only one image in this section
  • Allows only JPG, JPEG and PNG formats
  • Controlled with Edit Investigation permission
  • Available only in Web for Injury/Illness and Near Miss Incidents
  • Controlled with existing SEWO report custom setting

PMP-56621 Sewo Report Enhancements

Existing System

  • Does not highlight impacted body part on the Diagram
  • Does not auto-populate sketch Image
  • Does not show "Long Term Countermeasure Effectiveness"

Purpose:

  • Make SEWO report more informative

Enhanced System:

  • Modified the fields and layout as requested by FCA
  • Merged “Sketch – Problem” and “Sketch – Phenomenon” into 1 field and renamed it as "Sketch"
  • Updated Body Part diagram to display squares for injured body part
  • Included column to display number of weeks for "Long Term Countermeasure Effectiveness"

PMP-56607 As an end user I would like to identify primary root cause for an incident

Configurable: Yes

  • Title: Show/Hide Primary Root Cause
  • Public / Private: Private
  • Default: Disabled in Product

Existing System

  • Does not have a way to identify the Primary Root Cause of the Incident when multiple root causes are identified

Purpose:

  • Provide ability to identify Primary Root Cause of an Incident out of many Root Causes

Enhanced System:

  • Has a field that populates all selected root causes and users can select one of them as Primary Root Cause
  • If there is only one Root Cause identified it will be by default selected

PMP-57120 As an end user I would like to capture Asset information as part of Investigation Details

Configurable: No

  • Title: NA
  • Public / Private: NA
  • Default: NA

Existing System

  • Does not have ability to capture asset information involved in the incident
  • Does not have a way to maintain Asset inventory

Purpose:

  • Provide ability to select Asset involved in an Incident
  • Manage Asset inventory

Enhanced System:

  • Has a look up value available where system admin can manage a centralized Asset Repository. Asset ID is the unique identifier and mandatory
  • Has a new section in Investigation Details named "Common Questions" where question common to all Incident Types can be configured
  • This section cannot be configured to be visible for specific Incident types
  • If a question is configured under this section, it will be visible for all Incident Types
  • Asset related fields are configured in this section for Cudd Energy
