PMP-6527 Combine the CF, 5 Whys, Root Cause Analysis and CAPA into one screen

Configurable: No

  • Title: NA

Existing System: 

  • As of now, different steps of investigation are in different screens and users need to navigate to multiple screens
  • There is no mapping between Contributing Factors, 5 Whys and Root Causes

Purpose: 

  • Combine Contributing Factors, 5 Whys and Root Causes steps in one screen to increase mapping visibility among these sections 

Enhanced System:

  • Contributing Factors, 5 Whys, Root Causes, Final Root Cause Statement and Action Item sections are displayed in one screen
  • Users can define contributing factors, write down 5 Whys analysis, mark them as Root Cause and then map it with applicable root cause category
  • No change in the Incident Summary screen
  • All existing functionality, business rules and configuration features are as is, except for business rule to mark respective section as complete
    • Contributing Factor Section
      1. As it currently happens, if one contributing factor is identified, the section will be marked as complete
    • 5 Whys Section
      1. Required number of Whys should be available for at least one of the evaluations
      2. For each Contributing Factor at least one Why statement should be identified as Root Cause
      3. Existing Evaluation custom setting should be considered (one of the below):
        1. Evaluation of AT LEAST ONE CF is mandatory
        2. Evaluation of ALL CFs are mandatory
        3. Evaluation of  AT LEAST ONE CF from each category is mandatory
    • Root Cause Statement section
      1. At least one Root Cause Category is identified
  • Additional column "Confirm" Checkbox next to each Why is provided in 5 Why section
    • It can be enabled/disabled using the public custom setting "Display Confirmed Checkbox"
  • Additional column "Management System Element" is provided in Root Cause Analysis section
    • It can be enabled/disabled using the public custom setting "Enable Management System Element"
  • Incident Summary report has been updated to reflect the mapping between Contributing Factor, Why marked as Root Cause and respective Root Cause category
  • A new report has been introduced in Report Central named "Incident Root Cause Analysis Detail Report"
    • Existing reports are as is. These will not show the mapping, but continue to display all data in respective columns
  • New data points are provided in IMS Cognos model to see mapped data as per new functionality

PMP-70111 As an end user, I want to capture additional adjuster information

Configurable: No

  • Title: NA

Existing System: 

  • Has few fields to capture adjuster-related information 
  • Character limit for adjuster note field is not sufficient

Purpose: Provide flexibility to capture more adjuster information and notes

Enhanced System:

  • Has four additional optional fields (Phone, Fax, Email and Address) to capture additional adjuster-related information
  • Allows user to enter up to 10,000 characters for Adjuster Notes
  • New fields are added as part of SOA grid column selection
  • If new fields are part of the view, those will also be included in export and email

PMP-70799 As an end user I want to see actual LT/RD days count in Manage Incident list screen

Configurable: No

  • Title: NA

Existing System: 

  • If OSHA 180 business rule is turned on, the system does not show actual days counts in Manage Incident or Case Management list screen

Purpose: Increase visibility to actual case days count for internal tracking

Enhanced System:

  • Two new columns are added in Manage Incident List filter screen column selection
  • Users can add these columns in their views as per need
  • These new columns are added only in "Case Management - Current Year" view by default

PMP-63532 As a system admin I want different options to assign as Management Review Owner

Existing System: 

  • No option to automatically assign injured employee's supervisor and supervisor's supervisor or a Group/Role as Management Review owner

Purpose: Assigning automatically injured employee's supervisor and supervisor's supervisor or a Group/Role as Management Review owner

Enhanced System:

  • Has option to assign injured employee's supervisor and supervisor's supervisor or a Group/Role as Management Review owner
  • Individual owners from the selected group/role will be assigned to Incident Management review
  • Any change in the group/role will not reflect for the existing management review. It will reflect moving forward

PMP-69573 As a System admin I would like to trigger notification to Supervisor of the user who created the Incident

Existing System: 

  • Does not provide the option to automatically notify the supervisor or supervisor's supervisor of user who created the incident so that they can further follow up or take next steps

Purpose: Provide ability to send notification to supervisor and supervisor's supervisor of users who create the Incident

Enhanced System:

  • Has option to configure supervisor or supervisor's supervisor of user who creates the incident as notification recipient

PMP-53311 Revision to Goal Setup feature

  • Has goal setup by Month
  • No option to configure yearly goal

Purpose: Make the feature consistent and meaningful

Enhanced System:

  • Allows to configure Yearly Goal
  • Monthly Goal value configuration is deprecated
  • All existing monthly Goal Setup configurations will be as it is in the back-end
  • However to show the yearly goal for existing setups, least goal value from all months is configured as yearly goal value

  • Comparison against goal value and previous period rate can be seen

Existing System: 

  • The existing MAP point widget does not provide details for every dot
  • Does not have drill through

Purpose: Provide more information on widget

Enhanced System:

  • The widget has been renamed as "Risk Point"
  • Displays additional Incident Information on mouse hover on every dot (not supported in IE 11. Works in all other browser including Microsoft Edge)
  • On click on the dot navigates to respective Incident's Summary screen
  • If user does not have permission for the incident type then navigation is stopped

PMP-69584 As an end user I want the ability to report incident for other location from FROI

Configurable: Yes

  • Title: Display Location Code
  • Public/Private: Public
  • Default: Disabled

Existing System: 

  • Users have to change the location from navigator to report incident for another location

Purpose: Flexibility to report incident for another location without leaving the already navigated location

Enhanced System:

  • Allows users to select a different location and report the incident (all incident types)
  • User need not navigate away from already navigated location
  • Incident created for other location will show in the manage incident list of the respective location and not in the navigated location's manage incident list screen 
  • In Employee pick list filter there are below exceptions:
    • Current location radio button will consider location selected in the navigator
    • Custom location radio button will consider location selected in the drop down
    • Add Employee tab will consider location selected in the navigator

PMP-48177 As an end user I want the ability to link an existing Assessment with Incident

Configurable: Yes

  • Title: Enable IMS Risk Assessment Verification
  • Public/Private: Public
  • Default: Disabled

Existing System: 

  • As of now there is no way to link an existing Assessment with an Incident or prompt user to create an Action Item if Assessment is not done

Purpose: Track effectiveness of a Risk Assessment by linking it to an Incident

Enhanced System:

  • Has a new section in Incident summary screen after Investigation section which allows to link existing Assessments to Incident
    • Assessment of current and previous year will be displayed in the pick list
    • User can further filter the list using inline search 
  • The section will be enabled once Incident status is "Detail Reported Completed" or "Investigation Report Incomplete"
  • It will be an optional section
  • Provides option to create an action item if an assessment does not exist

PMP-48174 As an end user I want the ability to link an existing Training with Incident

Configurable: Yes

  • Title: Enable IMS Training Verification
  • Public/Private: Public
  • Default: Disabled

Existing System: 

  • Does not have functionality to link an existing Training with an Incident or prompt user to create an Action Item if Training does not exist

Purpose: Track effectiveness of Training by linking to a related Incident

Enhanced System:

  • Has a new section in Incident summary screen after Investigation section which allows to link existing Training to Incident
  • User can further filter the list using inline search
  • The section will be enabled once Incident status is "Detail Reported Completed" or "Investigation Report Incomplete"
    • The incident should have an individual involved (any personnel type except for Visitor and Unsupervised Contract employee)
  • It will be an optional section
  • Provides option to create an action item if training is inadequate
