Blank COC Form

Step-by-step guide

1. Click on the Module icon.
2. Click on the Industrial Hygiene menu item.
3. Click on the Blank COC Form menu item.
   
   
   
   
4. Select the Lab Name.
5. Click on the Run Report button.
   
   
   
   
6. The Blank COC form will generated in another window.
